what are your hours?

Our office hours are Mon-Thurs 8am-5pm. We are closed on Fridays, major holidays, and from Dec 23-Jan 2 every year.

Do you provide therapy?

We do not provide stand-alone therapy. Although you will receive some brief therapy regarding your particular diagnoses and treatment during your appointment, we primarily prescribe medication and recommend that every person seek additional therapy services with a qualified therapist.

What ages do you treat?

We are licensed for all ages but are not currently accepting patients over the age of 64.

Do you take insurance?

We are in network with : Aetna, AvMed, Cigna, Tricare, Humana, Optum, Oscar Health, UMR, United Healthcare, UHC | UBH and BCBS/ Florida Blue.

We are not in network with Medicaid or Sunshine Health and are not currently accepting medicare.

We also offer out of network services and are happy to provide you with documentation so that you may request reimbursement from your insurance company

how much are appointments?

New patients are $275 and follow ups are $175 by credit or debit only.

Do you have sliding scales or payment plans?

We do not have sliding scales or payment plans. Payment is expected at the time of appointment, and we do require a credit card on file (including insurance) to book an appointment.

Do you do testing or treat ADHD (Attention Deficit hyperactivity Disorder)?

We absolutely do diagnose and treat ADHD. We base our diagnoses on a clinical assessment utilizing DSM criteria. Depending on age, symptoms, and clinical presentation, we may also use a variety of screening tools.

Do you do testing or treat autism spectrum?

We absolutely do diagnose autism spectrum for patients as young as 18 months. The types of assessments used for diagnosis depend on age, symptoms, and clinical presentation.

how soon can i get an appointment?

We usually have availability within a few weeks.

do you treat Depression?

Absolutely! Depression is one of our practice specialties.

do you treat anxiety?

Yes, we specialize in anxiety disorders including panic disorder without the use of "rescue" medications such as xanax, ativan, or clonazepam.

do you treat PTSD (Post traumatic Stress Disorder)?

There is no medication that heals traumatic experiences, but we can treat the associated symptoms such as depression, anxiety, panic attacks, hyper vigilance, and nightmares. We strongly recommend therapy by a qualified therapist such as EMDR (Eye Movement Desensitization and Reprocessing) for PTSD in conjunction with medication. We do not do EMDR at Evolution. We can recommend EMDR therapists.

do you treat OCD (Obsessive Compulsive Disorder)?

Yes, OCD is an anxiety based disorder and is treated much the same as other anxiety disorders.

do you treat personality disorders (IE: Borderline)?

There is no medication that treats personality disorders. We recommend DBT (dialectical behavioral therapy) with a qualified therapist. We do not provide DBT therapy at Evolution. We CAN treat the associated symptoms of depression and anxiety.

do you treat bipolar disorder?

Although we are qualified to treat bipolar disorder, we do recommend that anyone with suspected or diagnosed severe bipolar disorder see a provider who specializes in bipolar disorder.

do you treat schizophrenia?

Although we are qualified to treat schizophrenia, we do recommend that anyone with suspected or diagnosed schizophrenia or other psychotic disorders see a provider who specializes in schizophrenia or other psychotic disorders.

do you prescribe?:

We prescribe all kinds of psychiatric medications. We do NOT prescribe benzodiazepines with the exception of safe tapers off

what is a psychiatric nurse practitioner?

Psychiatric nurse practitioners (PMHNPs) first become registered nurses (RNs) with Bachelor's degrees and usually spend several years working as nurses before attending graduate programs. PMHNP programs utilize RN education and field training as their general medical foundation and PMHNP programs are extremely focused on the specialty.

PMHNPs are highly qualified but they are not physicians or medical doctors and do not have the same amount of extensive training in general medicine.
