**PLEASE NOTE: You are responsible for any payment not covered by your insurance. If you are unsusre of your insurance coverage, please contact your insurance company. Also, please note that deductibles reset each new calendar year and if your deductible for the year has not been met, you will be responsible for the full visit amount (rates below) which will then be applied to your deductible for the year.
