About us

We are a team of highly qualified and experienced psychiatric nurse practitioners who are dedicated to providing exceptional care to our patients. Advanced practice nursing aspires to holistically treat patients with compassion, respect, and kindness.

We celebrate diversity of all kinds. We are affirming and knowledgeable in LGBTQia+ mental health including gender diversity, polyamory, and kink. You will always be treated with dignity and respect at Evolution.

To schedule an appointment:

To schedule an appointment, please click the link below and choose from the providers and appointment types. Once you choose your provider and appointment type, you will be directed to our intake form. This form takes 10-15 minutes to complete and is required prior to scheduling. Once the intake form is submitted, you will then be forwarded to the scheduling page where you may choose your appointment or you may call our office for assistance.. If you do not have time to complete the entire form in one sitting, your progress may be lost and you may be required to start over. You will be required to submit a copy of your legal ID, you insurance card (if applicable), and a credit card for us to keep on file for copays. Your card is not charged until your appointment (up to 24 hrs in advance). For any questions, please contact our office by phone or email.

Friday: CLOSED

We are closed on all major holidays and from Dec 23-Jan 2 every year. 

We do not offer on call services. For urgent issues, please contact your primary care provider or an urgent care center. For emergencies, please call 911 or go to your nearest emergency room or crisis unit.
